This a high-level overview and informational doc that details the importance to submit new, relevant and thorough evidence for review. Ensure you have your rating decision letter and any other supporting docs ready to upload (Med docs, Buddy Letters, DD214, etc). *Please do this when you are ready to set aside 10-15 min. Your answers must be thorough and complete. Take this step very serious as it provides the detail we need for the doctors. It will also provide fees and next steps.
The action plan assessment will determine what the doctors will and will not be able to provide an opinion on. It will provide a good amount of detail. Once this fee is paid, you will receive an agreement for review and signature.
This is the actual opinion report in which is compliant, neutral, relevant and thorough. The opinion will assess the diagnosis and event, injury or disease in-service. Any positive opinions will be supported by competent rationale supported by professional medical sources. Each opinion is also reviewed by an accredited VA attorney for compliance, neutrality, quality, and legal standing. **Don't pay this fee until an action plan assessment is received, reviewed and approved by you.
As an example, a service connection and VA disability rating for PTSD requires a diagnosis of PTSD (preferably by a licensed Psychologist), a stressor event in-service, and an independent medical opinion (IMO) that will provide the nexus or "connection" between the diagnosis and stressor event in-service. Just because you have a diagnosis and a stressor event, does not necessarily mean a doctor will the connect the two.
The Nexus Statement
A nexus statement is the actual medical opinion statement within an IMO . A nexus statement is what connects a diagnosis to an event, injury, or disease in service. We analyze the facts as neutral third party to help determine if there may be possible medical condition caused by military service. For your condition to be service connected it must meet the following three key items:
1) A current diagnosis
2) An event, injury, or disease while in service
3) A nexus statement by way of an IMO

Key Items
1) A Current Diagnosis
A current diagnosis from a medical professional is an essential component in the creation of a nexus statement. It is preferred that your diagnosis come from a licensed doctor or mental health professional (depending on your condition). Your symptoms must be severe enough as per VA regulations to receive payment. We are tasked with reviewing all medical evidence that you provide, which will be used to create a comprehensive file for a doctor to review. Simply put, it's not enough to say my back hurts or my head hurts. You need these symptoms officially diagnosed. Meaning your back pain can be due to arthritis or muscle strain, or your head pain may be due to headaches. As simple as they may be, each must be diagnosed.
2) An event, injury, or disease while in service. (Alternate theories such as secondary and aggravated theories can also be used in service connection.)
3) A Nexus - This is just a fancy way to say "connection".
Items 1 and 2 are used together or rather "connected" to create your Nexus Statement. Without either one it is not possible to establish the “connection”. Only a medical professional can provide a nexus. Preferably a doctor. This is where our doctors analyze your file to determine the connection between your diagnosis and event, injury, or disease while in service. We cannot provide you with a diagnosis.
